Fit notes
7 days off sick or less
- You may need a sick or ‘fit’ note if you have been off work for 7 days or less your employer should not ask for medical evidence that you've been ill. Instead, they can ask you to confirm that you've been ill.
- You can self-certify by filling in a form yourself when you return to work.

More than 7 days off sick
You can request a fit note via Klinik, our Online Portal
- We will give short-term sick notes based on our knowledge of your health or medical condition we are involved.
- We cannot give sick notes for care obtained from other services. If you are being treated in the hospital, only the doctor/specialist who attends to you can give you a sick note based on their knowledge of your health. The doctor will decide how long you need to stay off work based on your health.

- Long-term sick notes for benefits or DWP must come from your specialist. If you are working, we also suggest talking to your HR or Occupational Health team for further support

Note that we don’t consider sick notes as urgent request and therefore be mindful of this
Also, don’t assume that because you have submitted a request, your sick note will be granted. It is a discretionary duty of the doctor to issue one

Requesting Medical Records/Reports
Patients can ask for your medical records. We usually process requests within 28 days.
How to Request
- Write a request and drop it in the admin mailbox at reception
- View your GP health record - NHS on the NHS App
Please don’t ask for extra information not already in your records, especially for insurance or similar reasons.
Requesting a Supporting letter or Third-Party Requests
If a third party (like an insurance company or lawyer) asks for your records:
- We need proof, like an official letter from them.
- We may also ask for information from your specialist or former GP to complete the request.
- We do not write “support” or “fitness” letters. Instead, we provide a copy of your medical records. The requesting party (like your school or employer) can use these to decide for themselves.
Please note that GP letters are official documents and must only include facts. We cannot include information that we cannot prove is true. We will refuse a letter if we cannot confirm what you’re asking us to write. We usually process requests within 28 days. As this is a non-NHs service, the patients may be charged processing fees for these non-clinical requests, this usually communicated at the point of collection.
